十一、小兒麻痺、腦性麻痺所引起之神經、肌
肉、骨骼、肺臟等之併發症者（其身心障礙等
級在中度以上者）。
（一）急性脊髓灰白質炎併有其他麻痺者

十二、重大創傷且其嚴重程度到達創傷嚴重程
度分數十六分以上者

十三、因呼吸衰竭需長期使用呼吸器符合下列
任一項者：
Long-term mechanical ventilation,
defined as one of the following:
1. Invasive mechanical ventilation
for 21 or more days.
（二）使用侵襲性呼吸輔助器改善後，改用非
侵襲性陽壓呼吸治療總計二十一天以上者
2. Invasive mechanical ventilation
followed by non-invasive ventilation,
with a total duration of 21 or more
days.
（三）使用侵襲性呼吸輔助器後改用負壓呼吸
輔助器總計二十一天以上者
3. Invasive mechanical ventilation
followed by negative pressure
ventilation, with a total duration of
21 or more days.
（四）特殊疾病(末期心衰竭、慢性呼吸道疾
病、原發性神經原肌肉病變、慢性換氣不足症
候群)而須使用非侵襲性陽壓呼吸治療總計二
十一天以上者。
4. Specific diseases, e.g., End stage
heart failure, chronic pulmonary
diseases, primary neuromuscular
diseases, chronic hypoventilation
syndrome, which require
non-invasive ventilation for 21 or
more days.

（一）因腸道大量切除或失去功能引起之嚴重
營養不良者，給予全靜脈營養已超過三十天，
且病情已達穩定狀態，口攝飲食仍無法提供足
量營養者。
（二）其他慢性疾病之嚴重營養不良者，給予
全靜脈營養已超過三十天，且病情已達穩定狀
態，口攝飲食仍無法提供足量營養者。
